Japan Heat Survival Kit: OS-1, Salt Tablets and Q-max
What Japan's heat shelf does: OS-1 is a regulated medical food, not a sports drink, and Q-max only measures the first second of touch. Plus WBGT vs heat index.
The distinction the US shelf doesn’t make
Japan spent July 2026 setting records nobody wanted. The country logged five consecutive days at 40°C or above — the longest such streak on record — and in the week of July 13–19 alone, 10,857 people were taken to hospital by ambulance for heatstroke, roughly 2.4 times the same week a year earlier.
That pressure produced a consumer shelf with no real US equivalent. The most useful thing on it is not a gadget — it is a legal category. In Japan, rehydration after fluid loss is regulated as medicine-adjacent, not as a beverage. Here is what is worth importing, what has a US substitute, and what is mostly marketing.
OS-1 is not a sports drink, and the label proves it
OS-1 (オーエスワン), made by Otsuka Pharmaceutical Factory, is licensed by Japan’s Consumer Affairs Agency as an individually-evaluated food for the sick (個別評価型病者用食品). That is a formal regulatory approval covering dehydration from diarrhoea, vomiting, fever, insufficient oral intake and excessive sweating. It is sold in drugstores next to the drinks, but it is not one.
The mechanism is sodium–glucose cotransport. Sodium and glucose are absorbed together in the jejunum and ileum by a shared transporter, and water follows passively. Get the ratio right and you move water into the bloodstream faster than plain water does; load it with sugar and you slow gastric emptying and can worsen things.
The numbers make the difference obvious.

Pocari Sweat carries less than half the sodium of OS-1 and roughly three times the sugar; Gatorade sits in the same broad class as Pocari. Calling either an oral rehydration solution is a category error.
The honest caveat: most of the time you do not need an ORS. If you are sweating through a hot afternoon and eating normally, water and food do the job. ORS earns its place when fluid loss is heavy or you are already symptomatic — and Otsuka’s own guidance frames OS-1 as rescue, with stated daily intake ranges (500–1000 mL/day for older children and adults) rather than casual drinking.
OS-1 is not distributed through mainstream US retail; American buyers get it through Japanese import sellers at import prices. The functional US answer is a domestic ORS.

Salt tablets: Japan sells them, NIOSH says don’t
塩分タブレット sit at every Japanese convenience store checkout in August. They are also the one item on this list where official guidance actively disagrees across borders.
NIOSH’s position is blunt: workers should avoid salt tablets unless a doctor has instructed otherwise, because normal meals and snacks restore electrolytes for most people. Japanese consumer guidance takes the opposite framing — that heavy sweating outpaces meals — and pegs tablets at roughly 0.1–0.2 g of salt each, aligned to the 0.1–0.2% saline concentration Japanese health guidance describes for fluid replacement. Widely-repeated Japanese figures like “no more than five a day” come from dietitians and retailers, not from a government standard, and we could not verify an official ceiling.
Two things are not contested. Over-supplementing sodium is a real risk, particularly for anyone with hypertension or kidney disease. And most salt tablets are predominantly sugar — Japanese dental and low-sodium sources put a single tablet at roughly a sugar cube. If you are not sweating heavily, you are eating candy.
Q-max measures one second, and that’s the whole story
接触冷感 (“contact cooling”) is the biggest category in Japanese summer bedding and apparel, and the number on the package is Q-max, measured under JIS L 1927.
Q-max is the instantaneous peak heat flux, in W/cm², when a warmed sensor first touches the fabric. Japan’s testing institutes are unambiguous about what that means: it is a momentary value. JIS L 1927 treats 0.100 W/cm² and above as having contact-cooling performance, while the retail convention has drifted to 0.2 and above as the bar worth advertising — and even at 0.2, the felt effect is modest.
What Q-max does not measure is whether the fabric keeps you cool. It says nothing about breathability, moisture transport or airflow. Japanese bedding specialists make the point directly: some products chase a high Q-max by packing synthetic fibres to very high density, which produces a genuinely startling first touch and then traps humidity, leaving you sweatier at 3 a.m. than a plain cotton sheet would.
Practical reading: use Q-max to break a tie between two otherwise similar products, never as the primary spec. Breathability and moisture transport decide the night. Contact-cooling bedding is also one of the few categories where the Japanese range has no US counterpart at the same depth or price.

Cooling towels are the honest version of the same idea and are freely available in the US. They work by evaporation, not contact — which means their performance collapses in still, saturated air. In humid stagnant heat they do very little; in moving air they work well.
WBGT: Japan measures heat differently, and now enforces it
The US heat index uses temperature and humidity, calculated for shade. WBGT (wet bulb globe temperature) combines three readings — outdoors typically 0.7 × wet bulb + 0.2 × globe + 0.1 × dry bulb — so it captures solar load and air movement as well. On a sunny, humid day the two can diverge sharply, which is why WBGT, not the heat index, governs athletic and occupational heat limits.
Japan runs national alerts on it. The Ministry of the Environment issues a Heatstroke Alert when forecast daily maximum WBGT reaches 33, and a Special Heatstroke Alert at 35 across all measurement points in a prefecture. The 2026 season runs 22 April to 21 October.
The regulatory gap is the striking part. Japan’s revised Ordinance on Industrial Safety and Health took effect on 1 June 2025, making heatstroke countermeasures a penalty-backed employer duty for work at WBGT 28 or air temperature 31 and above lasting one continuous hour or more than four hours a day. Employers must establish a reporting route for suspected cases, define response procedures, and brief workers on both. Non-compliance carries up to six months’ imprisonment or a fine up to ¥500,000.
OSHA’s comparable rule is still not final. The proposed Heat Injury and Illness Prevention standard was published in August 2024; comments closed January 2025, hearings ran to July 2025, post-hearing comments closed October 2025. As of 2026 it remains on the regulatory agenda with no finalization date. Japan has enforcement; the US has a proposal — which is why a WBGT meter is a reasonable buy for US coaches, site supervisors and anyone training outdoors. It is the metric the standards actually use.
Wearables and fans, briefly
Two more categories, covered in depth elsewhere on the site. Fan-cooled jackets (空調服) are a mature Japanese industrial product working on the same evaporative principle as cooling towels — moving air across sweat. Neck-worn Peltier coolers are the fastest-growing consumer category and the one with the widest gap between marketing and measured effect.
If someone is in trouble
CDC/NIOSH draws the line clearly. Heat exhaustion presents as heavy sweating, cold clammy skin, weak fast pulse, nausea, cramps, dizziness or fainting. Heat stroke presents as confusion, altered mental status, slurred speech, loss of consciousness, very high body temperature, and skin that may be hot and dry — the body’s cooling has failed. Heat stroke is a medical emergency: call 911, move the person to shade, and begin active cooling while help arrives. No drink on this page treats heat stroke. Follow CDC/NIOSH or local emergency guidance, not a product label.
Summary
Japan’s heat shelf is worth studying because its best ideas are regulatory, not technological. OS-1 works because it is held to a composition standard. WBGT works because it measures the thing that actually kills people. Q-max is the counterexample — a real standardised measurement repurposed into a marketing number for a sensation that lasts one second. Buy the ORS, buy the meter, ignore the Q-max race, and check whether a cooling product depends on evaporation before taking it somewhere humid and still.
