Smartwatch Health Sensors Explained: FDA-Cleared vs Wellness (2026)
ECG, blood pressure, SpO2, sleep apnea alerts: which smartwatch health features are FDA-cleared, which are wellness estimates, and what each can tell you.
Short answer: every health feature on a smartwatch falls into one of two buckets — FDA-regulated features cleared for one narrow job (ECG, AFib notifications, sleep apnea notifications, Apple’s hypertension notifications), and wellness estimates that are not regulated at all (SpO2 on most watches, sleep stages, stress, skin temperature, Samsung’s blood pressure numbers). Both buckets are useful. They are not the same thing, and the marketing rarely tells you which one you’re looking at.
This guide sorts every major sensor claim on 2026 watches into the right bucket, using what the FDA and the manufacturers themselves have published — and explains what each feature can and cannot tell you.
FDA-cleared vs wellness feature: the one distinction that matters
When a watch feature is FDA-cleared (via 510(k)) or FDA-authorized (via De Novo, the pathway for genuinely new device types), the FDA has reviewed evidence that it does one specific, narrowly defined job — for a specific population, with specific exclusions. Note the vocabulary: cleared and authorized are the accurate words for these features. “FDA-approved” is a different, more demanding pathway, and no mainstream smartwatch feature uses it.
Everything else on the watch is a general wellness feature. That category got a formal update on January 6, 2026, when the FDA finalized a revised version of its General Wellness: Policy for Low Risk Devices guidance: a non-invasive product may now display estimates of physiological parameters such as blood pressure or oxygen saturation without becoming a regulated medical device — as long as it avoids disease claims and diagnostic thresholds. The FDA also warned that outputs suggesting medical use or guiding clinical decisions are not low-risk. This 2026 change is exactly why Samsung can ship blood pressure estimates in the US without clearance, and it makes reading the fine print more important, not less.
Neither bucket is “fake.” Wellness estimates are genuinely useful for spotting trends. But a cleared feature has defined performance for a defined purpose; a wellness estimate has whatever accuracy the maker chose not to make claims about. ECG on a smartwatch: cleared, single-lead, and deliberately narrow
The ECG apps on Apple Watch, Galaxy Watch, Garmin and Pixel Watch are all regulated in the US — Apple’s was authorized through the FDA’s De Novo pathway back in 2018, Garmin’s received 510(k) clearance (K221774) in January 2023 as an over-the-counter feature, and Samsung’s and Google’s apps are likewise FDA-regulated. They record a single-lead ECG — Apple describes it as similar to a Lead I recording, one of the twelve leads of a clinical ECG.
What they are cleared to do is narrow: classify a 30-second recording as sinus rhythm, atrial fibrillation, high or low heart rate, or inconclusive. Apple states plainly that the feature cannot detect a heart attack, blood clots, a stroke or other heart conditions. If you have chest pain, pressure or other symptoms that worry you right now, the correct tool is emergency services — not another recording.
Eligibility differs by brand, which surprises people: Apple and Samsung set the minimum age at 22, and Samsung additionally excludes people with pacemakers or implanted defibrillators, and people with known arrhythmias other than AFib. Model coverage differs too — Apple’s SE line does not include the ECG app, Samsung’s current Health Monitor page lists Galaxy Watch7 and later, and Garmin’s cleared app spans a wide range including the Venu 3 and 4, fēnix 8 and 9 series, Forerunner 970 and Enduro 3.
AFib notifications: three related features that get mixed up
Three Apple features sound alike and do different jobs — worth separating because reviews constantly blur them:
- Irregular rhythm notifications (FDA-authorized, 2018) run in the background using the optical sensor and notify you only if repeated readings suggest AFib. It’s a screening prompt for people not diagnosed with AFib.
- The ECG app is the on-demand recording you actively take, described above.
- AFib History (FDA-cleared, 2022) is the reverse audience: it is only for people already diagnosed with AFib, and tracks the estimated percentage of time spent in the rhythm so they and their clinician can watch the trend.
Google’s Pixel Watch line adds a distinct cleared feature: Loss of Pulse Detection (FDA-cleared in February 2025, on Pixel Watch 3 and later) can detect the absence of a pulse and escalate to emergency services. Google excludes users under 22 and anyone a clinician has assessed as high-risk for sudden cardiac arrest, and states it is not a substitute for prescribed cardiac monitoring.
Smartwatch blood pressure: three very different things wearing one name
“Blood pressure on your wrist” is where the cleared-vs-wellness line matters most, because three fundamentally different technologies share the phrase:
1. A real cuff on your wrist (FDA-cleared measurement). The Omron HeartGuide, FDA-cleared and on the US market since January 2019, inflates an actual cuff built into the band and takes an oscillometric measurement — the same principle as an arm cuff. It runs around $499 street at the time of writing. The category also gained a second cleared path: Aktiia’s Hilo Band received FDA 510(k) clearance in July 2025 as the first cuffless blood pressure monitor cleared for over-the-counter use, estimating from an optical sensor, with US consumer availability stated for 2026. 2. An optical estimate (general wellness, not FDA-cleared). Samsung’s blood pressure feature began rolling out in the US on March 31, 2026 — for Galaxy Watch4 and later — under the FDA’s general wellness policy rather than a clearance. Samsung itself says it is not intended to diagnose, treat or prevent any condition. Practically, you must calibrate it against an upper-arm cuff and re-calibrate every 28 days. It can be an interesting trend signal between real measurements; it is not a blood pressure monitor.
3. A pattern notification with no numbers at all (FDA-cleared, but not a measurement). Apple’s hypertension notifications, cleared by the FDA on September 11, 2025 (K250507) and shipped with watchOS 26 on Apple Watch Series 9/Ultra 2 and later, never display a blood pressure value. The watch analyzes how your blood vessels respond to heartbeats over 30-day windows (you need to wear it at least 14 of those 30 days; detection runs in the daytime) and notifies you if the pattern is consistent with hypertension. It’s for adults 22 and over who are not pregnant and have not been diagnosed with hypertension — and a notification means “check with a real blood pressure cuff and talk to a clinician,” nothing more.
If someone tells you their smartwatch “measures blood pressure,” ask which of the three they mean. In the US in 2026, only category 1 gives you cleared numbers.
SpO2 and blood oxygen: estimates, not a pulse oximeter
Almost every watch now shows a blood oxygen percentage, and almost none of it is regulated. Apple’s Blood Oxygen feature is explicitly “not intended for medical use,” and Garmin’s Pulse Ox tracking carries the same disclaimer — Garmin states it is not a medical device intended for the diagnosis, treatment or prevention of disease. These are wellness estimates for spotting trends in sleep and recovery.
One US-specific wrinkle: because of the Masimo patent dispute, Apple Watches sold in the US had the feature disabled in early 2024, then restored in August 2025 in a redesigned form where the sensor data is calculated on the paired iPhone and shown in the Health app’s Respiratory section. Sleep apnea notifications: the newest cleared category
This one is genuinely new and genuinely regulated. Samsung’s sleep apnea feature received De Novo authorization in February 2024 — the FDA’s first for a smartwatch — on Galaxy Watch7, and Apple’s version was cleared in September 2024 for Apple Watch Series 9/Ultra 2 and later on watchOS 11.
Both work over multiple nights rather than diagnosing from a single sleep. Samsung requires at least two nights of four-plus hours of sleep tracking within ten days; Apple’s approach uses the accelerometer to detect breathing disturbances and evaluates every 30 days. Both target signs consistent with moderate-to-severe obstructive sleep apnea in adults who have not been diagnosed (Samsung sets the bar at 22+, Apple at 18+), and both stop at a notification. Sleep stages, stress, skin temperature: always wellness, on every brand
Three features that are wellness estimates on every watch, no exceptions worth noting:
- Sleep stages are inferred from movement, heart rate and heart-rate variability — not from the brain-wave measurements a sleep lab uses. Samsung’s own fine print says its sleep features are intended for general wellness and fitness, not for the detection, diagnosis or treatment of any condition or sleep disorder. The nightly stage breakdown is soft; the multi-week trend is the useful part.
- Stress scores are heart-rate-variability math, not a psychological or clinical assessment. Useful as a “rougher week than usual” signal, nothing more.
- Skin temperature tracks change from your personal baseline, mostly during sleep. It is not a thermometer and cannot tell you whether you have a fever — it is used for things like cycle tracking and spotting deviations.
None of this makes them worthless. It makes them trend instruments. If a reading worries you
The boring, correct protocol, applicable to every feature above:
- A notification is not a diagnosis. These features are cleared to flag signs and prompt a conversation. False positives are expected and priced in.
- No notification is not an all-clear. The watch does not catch everything, and most features exclude whole groups of users by design.
- Take the data to a clinician instead of interpreting it alone. An exported ECG PDF or an AFib History trend is genuinely useful in an appointment; a night of anxious re-testing is not.
- Symptoms outrank sensors. Chest pain, fainting, severe shortness of breath — call emergency services. Do not stop to take a recording.
