Warmth is a targeting problem
A cold bedroom in January presents a choice that most people get backwards. The thing that needs to be warm overnight is one person, occupying roughly fifteen square feet of bed. The default solution warms the entire room — every cubic foot of air, the walls, the furniture — to accomplish that.

Over a 30-night month, the same assumptions give roughly 4 dollars for the bedding and roughly 61 dollars for the heater. These are illustrative numbers, not measurements — your rate, your model, and your settings will move them.
Two honest caveats. A heater with a thermostat cycles its element off, so a real night rarely runs the full 12 kWh; and heated bedding on a low setting draws less than the 100W assumed here. But even if the heater’s element runs only half the night, the gap is still several-fold, and the direction never changes: watts spent on room air are mostly spent on the room.

Under you or over you: the pad and the blanket are different tools
The two products get shelved together and cross-shopped as if interchangeable. Mechanically they are not.
A heated mattress pad goes under the fitted sheet, beneath you. Its heat has nowhere to go but up — through the sheet, through you, into the pocket of air your comforter traps. Nothing about your sleeping position folds it, bunches it, or slides it off, because it is strapped to the mattress. What the safety warnings actually say
Heated bedding has a safety reputation left over from an earlier product generation, and the honest version of the story is about product condition and instructions, not about the category being dangerous.
Buy listed products — a UL or ETL mark means the design was tested to the applicable safety standard. Retire old units: the recurring hazard in agency guidance and manufacturer literature is aging blankets with worn, cracked, or repaired wiring, and a blanket that predates its own care tag has earned replacement.
